Admitting/Outpatient Registration Office is open from 6:30 a.m. to 5:00 p.m. Monday through Friday. Before or after those hours you may go to the ER Registration Office which is open 24 hours a day 7 days a week. All patients’ need to bring the following items to expedite your Registration process: Doctor's written or printed order with Diagnosis (unless ER patient) Drivers License Insurance Card Any questions that you may have, you can call the Admitting/Outpatient and ER Registration at 407-498-3737.

This notice describes how your medical information may be used and disclosed and how you can get access to this information. Please read it carefully. The privacy of your medical information is important to us.

What Should I bring?

  • Contact Information ~ You will be asked to provide your social security number, address and telephone information.
  • Insurance Information ~ If you have an insurance card, please bring it with you. To properly bill your insurance carrier(s) for services rendered, we will need the name of the carrier (company), their complete mailing address, your policy’s group number and your personal identification number. If your insurance carrier requires a referral form, please bring it with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate cards as well.
  • Medical History ~ If your visit is for a second opinion or on a physician’s referral, you may need to bring medical records, x-rays, results from lab tests, pathology slides, or other materials. Your physician may have sent these to us already – be sure to ask your doctor before your visit.
  • Advance Directive ~ If you have an Advance Directive (a living will and / or durable power of attorney for healthcare) please bring it with you.
  • If you have an extended stay you can bring the following things with you:
  • Personal toiletries, bathrobe, slippers or socks
  • Any medications you usually take
  • Your glasses, dentures, hearing aid, etc.
  • Any written instructions your physician may have provided A list of your current prescriptions is also helpful. Be sure to write your name on the containers and inform your nurse.
  • What Should I Leave at Home?
  • We ask that you leave all valuables at home or with a family member when you come for outpatient procedures or inpatient care. We are not responsible for personal items that are lost or stolen on our premises.
  • Jewelry & Other valuables
  • Money & Credit Cards
  • A lot of money or extra credit cards are not necessary. However, you may want a few dollars to buy a snack.
  • Electrical Appliances ~ Please do not bring your hair dryer, electric shaver, radio or other electrical appliances.
